Awake Chin & Neck Liposuction
Precision Submental Contouring
Because the chin and neck involve fine, delicate anatomy, this is one of the most technique-sensitive areas we treat, and one where appropriate patient selection matters more than almost anywhere else on the body. Not every double chin is the same, and not every double chin should be treated the same way.
At Highlander Dermatology and Surgery Center, we’re proud to offer awake chin and neck liposuction, a precise, physician-performed treatment for stubborn fullness beneath the chin and along the jawline.
About Awake Chin & Neck Liposuction
Fullness beneath the chin, often called a “double chin,” isn’t always caused by fat alone. In our consultations, we evaluate three separate factors that can each contribute to submental fullness. Skin laxity, underlying muscle changes, and individual anatomy can all influence the appearance of the jawline and neck.
During your consultation, we assess your skin quality, fat distribution, jawline anatomy, and neck contour to determine which factor, or combination, is creating your fullness, and which treatment will actually address it. This distinction is the single biggest factor in whether awake liposuction is the right choice for you.
Areas We Treat
Submental Area (Beneath the Chin)
The most commonly treated area, and the one most people mean when they describe a “double chin.” A localized pad of fat beneath the chin is removed through a fine cannula to reveal a cleaner, more defined transition from chin to neck.
Jawline
Fat and mild skin laxity along the jawline can blunt its definition, making the face look softer or wider than a patient’s underlying bone structure. Liposuction along the jawline sharpens this transition, often the change patients notice first.
Jowls
Jowls, the fullness that develops along the lower cheek and jaw as skin loses elasticity, are usually a mix of fat and skin laxity rather than fat alone. Where fat is a meaningful contributor, awake liposuction can reduce that fullness; where skin laxity is the primary driver, we typically recommend pairing it with FaceTite or discuss whether a surgical option is more appropriate
Neck
Fullness that extends down the neck, beyond the immediate chin area, is treated with the same tumescent technique, contouring the neck to better match the jawline above it. This is also the area most likely to benefit from added skin tightening, since neck skin tends to show laxity earlier than the chin itself.
Face (Cheeks & Lower Face)
For patients with more diffuse fullness across the lower cheeks, chin, and jawline, treatment can be extended to create a more proportionate, contoured lower face, always evaluated individually, since over-removal in the cheeks can age the face rather than refine it.
Every treatment plan is customized to the area that is actually contributing to your concern, which is why a consultation matters as much as the procedure itself.
How Awake Chin & Neck Liposuction Works
If mild to moderate skin laxity is present, liposuction may be combined with radiofrequency-assisted skin tightening (RFAL) to improve contraction and further define the jawline and neck. The technology used is selected based on the extent and location of laxity.
FaceTite / FaceTite Turbo
FaceTite is typically for patients with skin laxity that extends beyond the chin into the broader jawline and neck. FaceTite uses the same internal/external bipolar electrode technology as BodyTite, scaled for the face and neck. It liquefies fat while heating the deep dermis to trigger collagen contraction, offering more substantial lifting and tightening than AccuTite alone, often the difference between a “refresh” and a more complete non-surgical jawline and neck rejuvenation.
AccuTite
AccuTite, InMode’s smallest RFAL handpiece, is designed for tight, detailed spaces like beneath the chin and along the jawline, delivering precise radiofrequency energy with minimal disruption to surrounding tissue.
Controlled radiofrequency energy helps contract tissue and stimulate collagen while allowing for precise treatment with minimal disruption to surrounding structures.
QuantumRF 10
QuantumRF 10 provides another targeted option for improving skin laxity throughout the face and neck. Its smaller applicator delivers fractionated radiofrequency energy to stimulate collagen and encourage tissue contraction after unwanted fat has been removed.
When appropriate, combining QuantumRF 10 with liposuction allows us to address both the underlying fat and the quality of the overlying skin.
Not every patient requires skin tightening after chin and neck liposuction, and no single technology is appropriate for every concern.
The appropriate technology, or combination of technologies, is selected based on skin quality, degree and location of laxity, anatomy, and desired outcome.
Candidates for Awake Chin & Neck Liposuction
Awake chin and neck liposuction may be appropriate for patients with a defined pocket of submental or neck fat that persists despite maintaining a healthy, stable weight. Ideal candidates may include individuals who:
- Have stubborn fat beneath the chin or along the jawline
- Are at or near a stable, healthy weight
- Have relatively good skin elasticity
- Want greater definition through the jawline and neck
- Prefer to avoid general anesthesia
- Have realistic expectations about treatment and recovery
Patients with unwanted fat and mild to moderate skin laxity may benefit from combining awake liposuction with FaceTite, AccuTite, or QuantumRF. A consultation with our team at Highlander Dermatology and Surgery Center will determine the most appropriate approach based on your anatomy and goals.

Frequently Asked Questions
Can chin and neck liposuction be performed while I'm awake?
Yes. Chin and neck liposuction can often be performed comfortably using local tumescent anesthesia, allowing you to remain awake throughout the procedure. This avoids the need for general anesthesia while still allowing precise contouring of the area beneath the chin and along the jawline.
How do I know if my concern is fat or loose skin?
Submental fullness can result from excess fat, loose skin, muscle laxity, or a combination of these factors. During your consultation, we evaluate your skin quality, fat distribution, jawline anatomy, and neck contour to determine what is creating the fullness and which treatment will give you the best result.
When does neck and chin liposuction work well?
It works best when the primary cause of fullness is excess fat rather than loose skin or muscle laxity, typically in patients who are at or near a stable weight, with a defined pocket of fat beneath the chin and reasonably good skin elasticity. When fat is the primary cause of fullness, liposuction alone can produce a noticeably sharper jawline and neck contour.
When should skin tightening be added to chin and neck liposuction?
If there is mild-to-moderate skin laxity in addition to unwanted fat, we may recommend combining liposuction with radiofrequency-assisted skin tightening, such as AccuTite, QuantumRF, or FaceTite, for more extensive laxity. This can help the skin contract more effectively around the newly sculpted jawline and neck.
How long is compression required after treatment?
A compression garment is typically worn beneath the chin for one to two weeks following treatment to help control swelling and support the area as it heals. Our team will provide a specific timeline based on your individual procedure.
What is recovery like after chin and neck liposuction?
Most patients experience swelling, bruising, and tenderness in the first several days, with light activities resuming quickly. Initial improvement becomes visible as early swelling resolves over the first couple of weeks, with continued refinement in jawline and neck definition over the following months.
When would you recommend a neck lift instead of liposuction?
Patients with significant loose skin, prominent neck bands, or more advanced aging of the lower face and neck are often better served by a surgical neck lift, which can address excess skin and muscle banding in ways liposuction alone cannot. We’ll discuss this openly during your consultation if it’s a better fit for your anatomy.
