In response to the COVID-19 pandemic Dr. Kleven and Courtney Papp PA-C are all offering a new version of TeleHealth virtual appointments on certain Fridays of each month. Dr. Straka is not offering virtual appointments at this time.
Effective MAY 18TH, 2020 we will have a new TeleHealth option for video appointments for which the only needed steps are to have a smart phone or tablet. A text message link will be sent to your phone number when it is time to join the video appointment. It is as simple as that! The system works with Android and iOS(Apple) devices and is completely secure.
ALL patients looking to have a virtual appointment must fill out the 'Virtual Appointment Consent' form.
NEW patients need to fill out the 'Patient Registration and Medical History Form', and (if applicable) the 'Medical Records Request' form. Click the link below to fill yours out now.
This simple to use system is ideal for acne, isotretinoin (previous accutane), eczema, psoriasis, and medication re-checks or refills.

Call our office to schedule your virtual appointment and fill out any necessary forms in advance. Forms are located in the 'Forms and Handouts' tab under 'Patient Resources'.
At the time of your appointment you will receive a text message. Just click the visit URL in the text message and you will automatically open your appointment with your provider.

Dr. Kleven and Courtney Papp PA-C offer virtual appointments alternating Fridays during the morning. Most insurance companies accept TeleHealth and your visit will be directly billed to your insurance. If you decide to pay out of pocket or do not have insurance coverage, the charge is $75.00 per visit.
Presently, you are not able to use online scheduling for TeleHealth, only for making in-office appointments.
